Achievement Day Authorization Forms

CMTA must be in compliance with Charleston Southern University policies in order to use the their facilities for Achievement Day. There are two different forms which teachers* must complete and mail to AD Co-Chair, Debbie Benson, by the AD Registration deadline.

*Note: Teachers who only participate in Virtual Achievement Day do not need to complete this paperwork. This only applies to teachers who will be on CSU campus working at Achievement Day.
